We make every effort to keep the cost of your medical care down.  You can help by paying at the time of your visit.  For your convenience, you may pay by cash, check, Visa or MasterCard.  Since insurance plans differ, we ask you to be aware of what you plan covers and of your responsibilities in regard to co-payments and deductibles.

HMO patients must bring the appropriate referrals and/or authorization from their primary physician.  If our office does not have the appropriate information at the time of service you will be obligated to pay at that time or have the option of rescheduling your appointment.  Remember, it is your responsibility to provide our office with your current and correct insurance cards and to know your coverage.

We participate with Blue Cross/Blue Shield, Medicare and most third-party payors.  Please inform us if you have Master Medical through Blue Cross.  We will submit your claim to Blue Cross if your payment is made at the time of service, as required.  Co-payments are to be made at the time of service.  Also, please notify us if your insurance changes.
